Community Care Grant!
Congratulations to our PACN practices that were awarded the PAMED Community Cares grant! This $4M grant to improve community care was made possible by the relationships that Highmark developed with the PACN in their True Performance Quality programs. More information about best utilization of the grant monies toward your approved quality initiatives will be forthcoming shortly.
Quality Results are “Paying”
Congratulations also to our many PACN practices that have begun to receive recognition from our payers for the quality care they provide their patients. The PACN quality pump has been primed and we are hopeful for more frequent distributions going forward. Please keep a keen eye out for messages and outreach from our Quality Team for tips to maximize your outcomes for your practice and our network. The Quality Team has also developed a helpful tiering grid to help the PACN and our practices determine where they currently are on the quality transformation journey. The most important thing is that our practices individually and collectively move forward on the pathway. And the most important step for improvement is engaging with our helpful PACN Quality Directors.
PACN-ACO
June was indeed a very productive month as the PACN also created an Accountable Care Organization (ACO) to apply for the 2022 Medicare Fee For Service Shared Savings Program (CMS FFS MSSP). This is another opportunity for our PACN practices with eligible Medicare lives to earn recognition for the great care that they provide their patients every day. Thanks to those eligible Medicare practices that already signed up for our PACN-ACO and we will share updates as available from CMS. Please let us know any questions about our participation in the upside only track for CMS 2022 MSSP.
Patient Satisfaction Surveys
We have launched a patient satisfaction survey which is being distributed to Capital Blue Cross patients seen between June 21 and July 30, 2021. PACN practices participating with CBC have been actively engaging their patients and soliciting valuable feedback about the care received at their practice. Patients participate by completing this brief 5-minute anonymous online survey or by completing a paper survey after their office visit.
Congratulations to John Manubay, MD, LLC for being the first practice to return 10 completed surveys!
Helpful Quality Tips
- BMI’s for children 3-17 years must include height, weight, and BMI percentile
- Annual Wellness Visits should include discussions about fall risk, physical activity, urinary incontinence, advanced care planning, pain assessment, depression screening and medication reconciliation
- Retake BP readings that are high at the end of the visit. Report the appropriate CPT II codes combining the best diastolic and the best systolic reading. Don’t wait for a return visit to document an end of year BP reading.
- Remember to bill CPT II codes especially for Blood pressures and HbA1C’s!!
