Pennsylvania’s LIFE program provides comprehensive medical, functional, social, pharmacy, transportation, and home-based services for eligible older adults through one coordinated interdisciplinary care team.
Some Patients Need More Support
Periodic office visits may not provide enough support for patients with complex medical, functional, and social needs.
The federal Program of All-Inclusive Care for the Elderly, known in Pennsylvania as LIFE (Living Independence for the Elderly), brings services together through a single interdisciplinary team.
For patients who qualify, LIFE can provide the support they need to continue living safely in their homes and communities.
Who May Be Eligible for LIFE?
Are age 55 or older
Live within a LIFE service area
Meet Pennsylvania's level-of-care criteria
Can live safely in the community with LIFE support
Important: Enrollment is voluntary. Final eligibility is determined by the Pennsylvania Department of Human Services.

What LIFE Means for Your Practice
For eligible patients, LIFE provides an organized transition to an interdisciplinary team responsible for coordinating the full continuum of care.
Following enrollment, patient attribution and associated care costs transition from the Medicare ACO to the PACE program.
PACN Handles the Outreach
No additional patient calls or paperwork are required from your office.
PACN will coordinate directly with potentially eligible patients and their caregivers.

Your practice will receive a list of potentially eligible patients through secure email.

Your Practice Provides Clinical Insight
Review the list and identify any additional patients you believe could benefit from LIFE.

PACN Handles Outreach
PACN contacts potentially eligible patients and caregivers and coordinates next steps.
Step 4
Eligibility & Enrollment Are Determined
Enrollment is voluntary, with final eligibility determined by the Pennsylvania Department of Human Services.

Enrollment in LIFE is voluntary. Final eligibility is determined by the Pennsylvania Department of Human Services. Following LIFE enrollment, responsibility for the patient’s care transitions to the LIFE/PACE program in accordance with applicable program requirements.
